BHSB investigates and responds to Critical Incidents that occur at public behavioral health programs in Baltimore City. A Critical Incident is any actual or alleged event or situation that involves, or creates a significant risk of, serious harm to physical or mental health, safety or well-being.
Reportable Critical Incidents include, but are not limited to:
- abuse
- neglect
- death
- serious injury
- assault
- medical errors
- medical emergency
- a missing person
- a suicide attempt
Providers are expected to report Critical Incidents to BHSB promptly. To report a Critical Incident, complete the BHSB Critical Incident form and send it to Critical.Incident@bhsbaltimore.org.
